COACHING AGREEMENT - (updated 12/14/24)

 

This agreement is between Cheryl Lynn Life Coaching LLC and the client who has scheduled coaching services from our website or directly with my life coach. 

 

COACHING SERVICES 

 

1. I am at least eighteen years old and have the mental capacity to make informed decisions to enter into this coaching agreement. 

2. As a client, I understand and agree that I am fully responsible for my well-being throughout the coaching relationship, including my choices and decisions. 

3. I recognize that coaching is not a substitute for therapy, counseling or psychotherapy. I understand that coaching does not diagnose or treat any mental disorders and will not use coaching as a replacement for any form of therapy.

4. If I am under the supervision of a professional therapist, I acknowledge that I have consulted with this person regarding the advisability of working with a life coach and that this person is aware of my decision to be coached.

5. I understand that coaching is not a substitute for professional advice in matters of finances, legal, medical, or business issues. I will seek the appropriate expertise I need and agree that all decisions in these arenas are exclusively my own. I take full responsibility for my decisions and my actions. 

6. I understand that coaching is a comprehensive process that may involve all areas of my life, my work, and my relationships.  I acknowledge that I decide how to handle these issues and implement my choices.  My coach is my guide, not my decision-maker.

7. I understand that what is discussed within the coaching relationship is considered confidential and will not be disclosed to other parties except as may be required by law. Also, if any evidence of illegal activity or the potential harm to myself or others is disclosed, my coach may have to inform the appropriate authorities and so be unable to maintain complete client confidentiality.

8. I understand that my coach will keep records of our coaching sessions, which may include video, audio and/or written notes, for her purposes to keep an accurate account of our discussions to better serve me. Details about my privacy and/or how my coach utilizes my information is covered in the Privacy Policy stated below as part of this Coaching Agreement. 

9. Disclaimer and Indemnification - I understand that life coaching is a collaborative process and serves as a supportive sounding board to help me explore ideas, identify solutions, and overcome hurdles that may impede my progress toward achieving my goals. Life coaching does not involve giving advice, providing direct answers, making decisions on my behalf, or dictating actions I should take. The role of the life coach is to act as a mirror, helping me to reflect, gain clarity, and make informed choices independently.

I acknowledge that I am fully responsible for any decisions I make or actions I take as a result of our coaching sessions. Accordingly, I agree to indemnify and hold harmless Cheryl Lynn Life Coaching LLC from any claims, liabilities, or damages arising from discussions during sessions or actions I may take after that.

PAYMENT / REFUND POLICY

Payment Terms: - Payment for all coaching services is due at the time of booking.

Rescheduling Policy: - You may reschedule your session without penalty if you provide at least 24 hours' notice. If you do not show up for your session you will forfeit the fee for that session.

Cancellation Policy: - You will receive a full refund for your scheduled session provided you notify me via email at least 24 hours before your session.

Refund Policy: - No refunds are allowed if you have not, via email notification, canceled or rescheduled your session at least 24 hours before your scheduled session.

PRIVACY POLICY

Your privacy is important to Cheryl Lynn Life Coaching LLC. This statement outlines how we collect, use, and protect your personal information when you use our life coaching services.

1. Information Collection
   We collect your name, contact information, and payment details when you schedule a session through Calendly. During coaching sessions conducted via Google Meet or Zoom, we may take notes using Otter. We also keep hard copy notes stored securely in a locked file cabinet.

2. Use of Information
   Your information is used solely to schedule and conduct coaching sessions, process payments through Stripe, and maintain records for our coaching relationship. We do not share your information with third parties, except as necessary to operate the tools and platforms we use to provide our services (e.g., Calendly, Stripe, Otter, Google Meet, and Zoom).

3. Data Security
   We take the security of your information seriously. Payment information is processed securely through Stripe, and any personal details are handled with care and stored securely.

4. Third-Party Tools
   Please note that the tools we use (Calendly, Stripe, Google Meet, Zoom, and Otter) have their own privacy policies. By using our services, you consent to the use of these tools as part of our coaching process.

5. Your Rights
   You have the right to inquire about the personal information we hold and request corrections or deletions as permitted by law.

Changes to This Coaching Agreement:

This agreement may be updated periodically. Any changes will be posted on this page with an updated effective date.
